Hope PACE (Program of All-Inclusive CARE for the Elderly) is a long-term care program for Seniors to remain in the community and avoid nursing placement.  We provide comprehensive healthcare under one roof, coordinated by a team of medical professionals.  Our goal is to keep our Participants in the community as independently and autonomously as possible, while striving to avoid placement in a nursing home.  Services and plans of care are individualized to meet our Participants' needs.  For those who qualify*, all services are provided at no cost, including zero premiums, copays, deductibles, and out of pocket expenses.  

*Those who are on Medicaid and live in the community.  Hope PACE assists potential enrollees in qualifying for Medicaid.  For those who don't qualify for Medicaid, there are associated premiums/private pay rates.
