PURPOSE:
The Case Manager for the Homeless Teen Transitional Living Program, also known as Rob’s Cottage Case Manager, supports teens and young adults experiencing homelessness by providing coordinated, compassionate, and individualized case management services. Guided by Youth Haven’s core values, this role ensures continuity of care through support, advocacy, goal setting, and discharge planning, while maintaining compliance with Department of Children and Families (DCF) and Council on Accreditation (COA) contractual and regulatory requirements.
Caring for our teens and young adults is demonstrated through the development of nurturing, safe, and supportive relationships that promote healing, stability, and self-sufficiency. The Case Manager acts with integrity by maintaining honesty, transparency, and strict confidentiality in all interactions and documentation. By being responsible to our community, this role partners with internal teams and external agencies to ensure clients receive comprehensive and coordinated services. Through a commitment to excellence, the Case Manager upholds the highest professional, ethical, and regulatory standards in documentation, compliance, and service delivery.
FUNCTIONAL RESPONSIBILITIES:
*Build emotionally safe, developmentally appropriate, and supportive relationships.
*Provide consistent guidance, encouragement, and advocacy to empower clients.
*Conduct comprehensive intake assessments for teens and young adults entering Rob’s Cottage.
*Develop, implement, and monitor individualized service plans and behavior management plans in collaboration with the Treatment Team.
*Promote the effective and efficient utilization of program and clinical resources.
*Establish, track, and evaluate goals that promote independent living, stability, and long-term self-sufficiency.
*Identify service gaps or emerging concerns and advocate effectively on behalf of our clients.
*Coordinate and organize on-campus and community-based services to meet the complex needs of teens and young adults.
*Collaborate closely with clinical staff, cottage staff, schools, and community organizations.
*Develop and maintain strong partnerships with external agencies.
*Maintain thorough, accurate, and timely documentation, including intake paperwork, case notes, discharge paperwork, and required data entry.
*Prepare client files for compliance reviews and audits.
*Adhere strictly to confidentiality laws, ethical standards, and organizational policies.
*Perform other duties as assigned.
EMPLOYMENT STANDARDS:
• Bachelor’s degree in social work or related field with at least one (1) year of
relevant experience working with teens or young adults.
• Strong verbal and written communication skills.
• Excellent assessment, organizational, and documentation skills.
• Demonstrated ability to build collaborative team relationships.
• Ability to manage conflict effectively and respond appropriately in crisis situations.
• Commitment to the organization’s mission and core values.
The pay range for this role is:
$23 - $25/ hour
